Liability Waiver & Release of Claims
By purchasing, registering for, attending, or participating in any class, membership, private lesson, workshop, event, social, rehearsal, fitness activity, or related service offered by The Tampa Dance Factory and The Tampa Dance Factory, LLC, I acknowledge and agree to the following:
1. Voluntary Participation
I voluntarily choose to participate in activities offered by The Tampa Dance Factory.
2. Assumption of Risk
I understand participation may involve risks including:
Slips, trips, or falls
Muscle strain or soreness
Physical contact with others
Equipment-related hazards
Negligence of other participants
Aggravation of existing conditions
Serious injury in rare cases
I knowingly and voluntarily assume all risks associated with participation.
3. Health Representation
I certify that I am physically able to participate or have consulted an appropriate medical professional.
4. Release of Liability
To the fullest extent permitted by law, I release, waive, and discharge The Tampa Dance Factory, LLC, its owners, instructors, employees, contractors, volunteers, affiliates, landlords, and agents from claims, damages, losses, or liabilities arising from my participation or presence on premises, except where prohibited by law.
5. Property Loss
The Tampa Dance Factory is not responsible for lost, stolen, or damaged personal belongings.
6. Conduct & Safety Compliance
I agree to follow studio rules and instructor directions. Unsafe, disruptive, intoxicated, or inappropriate behavior may result in removal without refund.
7. Photo & Video Release
I understand classes, events, and sessions may be photographed or recorded.
I grant permission for my likeness, voice, or image to be used for lawful promotional, educational, or business purposes unless I provide written notice in advance.
8. Emergency Care
If an emergency occurs, I authorize The Tampa Dance Factory to seek emergency assistance on my behalf if reasonably necessary. I understand I am responsible for related costs.
9. Binding Effect
This waiver is binding upon me, my heirs, family members, estate, representatives, and assigns.
10. Severability
If any portion of this waiver is unenforceable, the remaining provisions remain in effect.
11. Acknowledgment
By registering, purchasing, signing electronically, checking an agreement box, or participating, I acknowledge that I have read, understood, and voluntarily agree to this Waiver.
