MINOR PARTICIPANT LIABILITY WAIVER

By registering a minor child for participation in any class, camp, workshop, event, rehearsal, fitness activity, or related service offered by The Tampa Dance Factory and The Tampa Dance Factory, LLC, I acknowledge and agree to the following:

1. Parent/Guardian Authorization

I certify that I am the parent or legal guardian of the minor participant identified above and have the legal authority to execute this agreement on their behalf.

2. Voluntary Participation

I voluntarily permit the minor participant to participate in activities offered by The Tampa Dance Factory.

3. Assumption of Risk

I understand that participation in dance and physical activities involves inherent risks, including but not limited to:

• Slips, trips, and falls
• Muscle strain, soreness, or fatigue
• Physical contact with instructors or other participants
• Equipment-related hazards
• Aggravation of pre-existing conditions
• Injuries that may require medical treatment
• Serious injury in rare circumstances

I knowingly and voluntarily assume all risks associated with the minor participant's involvement.

4. Health Representation

I represent that the minor participant is physically able to participate in dance activities or that I have consulted an appropriate medical professional regarding participation.

5. Release of Liability

To the fullest extent permitted by law, I release, waive, and discharge The Tampa Dance Factory, LLC, its owners, instructors, employees, contractors, volunteers, affiliates, landlords, and agents from any claims, demands, damages, losses, costs, or liabilities arising from or related to the minor participant's participation or presence on the premises, except where prohibited by law.

6. Property Loss

The Tampa Dance Factory is not responsible for lost, stolen, or damaged personal property belonging to the participant or guardian.

7. Conduct & Safety Compliance

I understand that participants are expected to follow studio rules and instructor directions. Unsafe, disruptive, or inappropriate behavior may result in removal from activities without refund.

8. Photo & Video Release

I authorize The Tampa Dance Factory to photograph or record my child and use such images, video, or recordings for lawful promotional, educational, or business purposes.

9. Emergency Medical Authorization

In the event of an illness, injury, or emergency involving the minor participant, I authorize The Tampa Dance Factory to seek emergency medical assistance when reasonably necessary if I cannot be reached immediately. I understand that I am solely responsible for any medical expenses incurred.

10. Binding Effect

This agreement is binding upon me, the minor participant, our heirs, family members, estate, representatives, and assigns.

11. Severability

If any provision of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.
